Care is delivered by a Registered Nurse or Licensed Practical Nurse, as prescribed by a physician and delivered according to an individualized plan of care.
Services are tailored to the individual’s medical needs and may be provided in the home or another appropriate setting.
Availability, required clinical orders, service eligibility, and payment arrangements are confirmed before services begin.
